Study links compulsive social media use to later ADHD symptoms in teenagers, especially boys

Teenagers who find they can’t keep their social media use in check are more likely to show increased ADHD symptoms the following year, according to a new study. The association was seen specifically with problematic social media use - the compulsive, difficult-to-stop pattern - and it was most apparent in boys.

The takeaway for parents and clinicians is practical: what seems to matter is not simply the number of hours a young person spends scrolling, but whether their use has crossed into something they struggle to control.

The impact was not evenly distributed. It appeared to hit boys harder than girls, and only during a fairly tight period in the middle of adolescence.

Thousands of teens

The research was led by Dr Jason M. Nagata, an adolescent-medicine physician at the University of California, San Francisco (UCSF).

His team used data from the Adolescent Brain Cognitive Development Study, a large, long-running US federal project tracking more than 11,000 children as they grow up.

The findings land at an anxious time for families. Around one in nine children and teenagers in the United States have been diagnosed with ADHD.

Current US federal figures indicate that total rose by roughly a million between 2016 and 2022 - the same period in which digital media became increasingly common among younger children.

The design was simple. From about age 12, the researchers followed up annually for five years.

Five years of data

Each year, the teenagers reported how much their social media use had become difficult to control.

At the same time, parents rated ADHD symptoms - including difficulty concentrating, restlessness, and impulsive behaviour - that characterise attention-deficit/hyperactivity disorder.

A key feature was how the comparison was made. Instead of contrasting one teenager with another, the researchers compared each teenager with their own earlier self.

They then examined whether years in which a teenager’s use was more compulsive than usual were followed by an increase in attention problems. During a mid-adolescent window - roughly ages 14 to 16 - that pattern emerged.

It is easy to overstate the scale. Over the five-year period, most teenagers’ scores remained low, so the results reflect relative rises and falls rather than a wholesale slide into addiction across an entire generation.

Reward and self-control

ADHD symptoms and compulsive social media use are thought to involve some of the same underlying mechanisms. Both are linked to impulsivity and a strong pull towards immediate reward.

At the same time, the systems that support self-control are still developing throughout the teenage years. That overlap is one reason researchers expected the two might shift together.

Social media platforms are designed to activate these reward pathways. Likes, notifications, and endlessly refreshed feeds can provide quick, unpredictable bursts of social reinforcement.

That kind of reinforcement can be especially difficult to resist while a teenager’s ability to hold back and regulate behaviour is still maturing.

Beyond screen time

Earlier work with high-school students in Los Angeles had already connected heavy digital media use with later ADHD symptoms.

However, that research emphasised how frequently teenagers used different platforms, rather than whether their use had become compulsive. That distinction sits at the centre of this new study.

Time spent online does not necessarily reveal whether a teenager feels preoccupied, repeatedly tries - and fails - to cut back, or becomes distressed when they cannot access their feeds.

Those experiences, rather than the clock, were what the questionnaire captured - and they were what aligned with the later increase in symptoms.

Sharper effects in boys

When the researchers analysed results by sex, the pattern shifted. The forward association - from compulsive use in one year to more attention problems the next - appeared in boys, but not in girls.

In boys, greater sensitivity to reward and a stronger tendency towards risk-taking may intensify that cycle. That does not mean girls were unaffected.

Among girls, those who had more persistent attention difficulties were also more likely to show more troubled social media use.

In other words, this looked like an ongoing background relationship rather than a year-by-year change.

Reward systems at work

A review covering more than two dozen studies has reported a similar overall picture: problematic use tends to track ADHD symptoms more consistently than screen time does.

The timing also aligns with what is known about adolescent brain development. In the mid-teen years, motivation for reward often outpaces the mental systems that restrain and regulate it.

By that point, most teenagers have social media accounts and use them frequently. Earlier in adolescence, fewer are engaged in that more intense way, while later they typically have more autonomy over their daily routines and habits.

The implications ahead

Before this study, much of the evidence connecting social media with attention problems came from one-off snapshots or simple measures of screen time. This analysis went further.

By following the same teenagers across multiple years and separating each individual’s year-to-year shifts from their more stable traits, the researchers still found that compulsive use tended to come before an increase in ADHD symptoms - largely among boys.

For parents, that reframes what is worth monitoring. Rather than focusing solely on hours, it may be more useful to watch for signs that use has become problematic.

These red flags include a teenager who cannot cut back, becomes agitated without access to their feeds, or allows schoolwork and sleep to deteriorate.

Small effects, useful clues

The observed effects were small, and the study cannot demonstrate that social media causes ADHD symptoms.

The results also depended on who reported what. Parents detected changes that the teenagers did not report in their own attention.

Even so, the findings offer clinicians a specific additional question to consider: not only how long a teenager is online, but how much control they feel they have over their social media use.
