Cough CPR: what social media gets wrong about heart attack and cardiac arrest

A strange tip from the late 1970s has resurfaced on social media: cough CPR.

In viral posts (over 270,000), the claim is that if you are on your own and having a heart attack, you can keep yourself alive by coughing in a steady rhythm to “keep the heart beating”. It is an attention-grabbing, cinematic idea, often presented as a life-saving hack - and it remains highly disputed.

Where cough CPR came from

Cough CPR did not begin as a home remedy. It stems from a technique used in very specific, closely monitored medical circumstances, such as certain hospital heart procedures.

In those controlled settings, clinicians may tell a patient to cough hard in order to briefly sustain blood flow - and therefore consciousness - when an abnormal irregular heart rhythm (arrhythmia) occurs.

Outside hospital, however, it is not designed to be used. Even so, it is frequently portrayed online as a universal solution.

Heart attack vs cardiac arrest: why the difference matters

A heart attack and a cardiac arrest are not the same emergency.

A heart attack happens when blood supply to the heart is obstructed because of a build-up of cholesterol and blood clots in the arteries that deliver the oxygen and nutrients the heart muscle needs to work properly.

A cardiac arrest occurs when the heart’s electrical system fails, meaning the heart stops pumping effectively.

Even if cough CPR did offer any benefit, it would only relate to particular arrhythmias - not to heart attacks, and not to a full cardiac arrest.

Cardiology and emergency medicine specialists have repeatedly stressed that cough CPR is not the right response for most emergencies.

The American Heart Association, the British Heart Foundation, the Resuscitation Council UK and comparable organisations around the world do not advise using it outside hospital.

Their concern is that depending on this unproven approach could waste precious time and delay treatments that are known to work - such as calling emergency services or starting standard CPR. In genuine emergencies, rapid medical care is essential.

Why the myth travels so well on social media

So what explains the renewed popularity of cough CPR?

Part of it is how viral content functions. Social media platforms tend to amplify posts that spark reactions, often giving emotionally compelling or surprising claims an advantage over careful, qualified explanations.

A promise that you can “save your own life” using a simple trick will inevitably spread faster than advice that involves less dramatic steps.

It also sits neatly within today’s wider health-misinformation environment. Alongside alternative cures and conspiracy-driven narratives, many people encounter medical guidance that sidesteps established scientific checks. Cough CPR fits that template: a tempting shortcut that is persuasive - but misleading.

Although cough CPR has some rationale in tightly supervised clinical situations, there is no strong body of research showing it is effective or safe for members of the public facing a cardiac emergency.

Instead, clinical guidance focuses on recognising symptoms, getting help quickly, and performing conventional CPR when required. Automated external defibrillators (AEDs) are far more effective at restoring a normal rhythm during cardiac arrest than any coughing manoeuvre could realistically be.

This wave of interest is a useful reminder of the difficulty of public health communication. It highlights the need for clear, practical education about what to do when someone’s life is at risk. In a life-or-death moment, dependable knowledge matters - not viral shortcuts.

What you should do instead

What should you do if you - or someone else - shows signs of a heart attack or cardiac arrest? The guidance is simple: call emergency services immediately.

If the person is unresponsive and not breathing, begin chest compressions at a steady pace of around 100 to 120 beats per minute until professional help arrives.

If an AED is available, use it exactly as the device instructs. These approaches are supported by decades of research and save countless lives every year.

Social media can be valuable for raising awareness, but it also demands scepticism from users.

Before you share or follow any medical advice, pause to check where it came from - and whether it applies in the real world.

Cough CPR may be a compelling story, but the truth is less dramatic and far more important: in an emergency, the best protection comes from proven methods, not myths.
